Abstract

Background: Osteoporosis is an important health problem among older women, while simple risk screening remains relevant in primary care. Body mass index (BMI), parity, and previous use of depot medroxyprogesterone acetate (DMPA) may be related to bone health through nutritional, reproductive, and hormonal pathways. Objective: To analyze the association of BMI, parity, and DMPA use with osteoporosis risk based on the Osteoporosis Self-Assessment Tool for Asians (OSTA) among older women at Cengkareng Primary Health Care. Methods: This cross-sectional correlational study involved 100 postmenopausal women aged ≥60 years recruited by consecutive sampling in June 2026. Data were collected by structured interview, anthropometric measurement, and OSTA calculation. Bivariate analysis used Fisher’s Exact Test, Chi-Square with Continuity Correction, or Pearson Chi-Square as appropriate, with α=0.05. Results: Fifty-three respondents (53.0%) were classified as high OSTA risk. All respondents with low BMI (8/8; 100%) were in the high-risk group (p=0.006). High parity was associated with high OSTA risk (25/30; 83.3%) compared with low parity (28/70; 40.0%; p<0.001). DMPA history was not significantly associated with OSTA risk (p=0.741). Conclusion: Low BMI and high parity were significantly associated with higher osteoporosis risk based on OSTA, while DMPA history was not. These findings support targeted osteoporosis risk screening in primary care.
